2) Your Email Address and telephone number

3) Location of requested Accommodation

4) Type of Accommodation: Villa / Apartment / Amsterdam Houseboat

5) Number of Double Bedrooms required

6) Number of Single Bedrooms required

7) Number of Beds required

8) Number of Persons in your party: Number of Adults / Children (under the age of 12)

9) Number of Bathrooms required: ensuite or semi-private

11) "Self-Catering" or Full-Service Accommodation (daily maid service)

12) If Full-Service, wIll you require a cook?

13) Any Special requirements: Local Guide / Airport-Train Station Transfer/ Airline -Train Tickets / Car/Van rental, etc.

14) Date of Arrival at Accommodation*

15) Date of Departure from Accommodation*

16) Any special requests, i.e. handicapped facilities

*Most accommodations are for a one-week minimum (Saturday to Saturday)
